Request an Absentee Ballot
- To request a vote-by-mail ballot, visit https://dos.fl.gov/media/708151/ds-de-162-eng-statewide-vote-by-mail-delivery-affidavit-form-final-adopted-eng-202405-fillable-pm.pdf,
- Click “Request a vote by mail ballot”
- Fill out the form, print it, and return it to your county’s Supervisor of Elections via mail, fax, or email attachment.
- You can also request in person at your Supervisor of Elections office, by phone, or on their website.

Visit https://www.myfloridaelections.com/Contact-your-SOE to find your county’s Supervisor of Elections website and find the early voting locations, dates, and hours of operation for your county.
Find Your Polling Location
Visit https://dos.fl.gov/elections/for-voters/check-your-voter-status-and-polling-place/voter-precinct-lookup/, select your county, and enter your information to find your polling location for Election Day.
